Your First Visit

OK, you’ve made your first appointment now what? It is common to feel nervous about visiting with a new doctor. Our support staff have been trained to help assist you with any questions you might have. Here is a step-by-step guide on what you can do and expect for your first visit.

Step 1 : Print New Patient Paperwork

We offer the ability to print out your New Patient Paperwork from home, before your visit. Having the ability to fill out your paperwork at home allows you to gather all the needed information without the rush so you can relax when you arrive for your appointment. Step 2 : Check- In

The day of your appointment has arrived, What now? For new patients, we recommend arriving no earlier than 15 minutes before your scheduled appointment time. Here’s a list of all the documents you need for check-in. 

Step 3 : Meet Your Care Team

During your appointment, you will meet with one of our medical assistants. They will gather more information for the doctors and be there to assist them with any procedures you may need for treatment. Your Doctor will diagnose and discuss treatment options with you along with helping create a custom treatment plan that is right for you. You may receive educational materials to help you understand the treatments you might receive or any necessary exercises that may help with pain management.

Step 4 : Custom Care Plan

After meeting your doctor and deciding on your care plan, You will have the option of contacting our supportive staff for any questions you may have regarding your treatment. You are not alone! We try hard to make sure you are comfortable and pain-free.

We accept a variety of insurance plans at our office, and we frequently update our participation with new insurance providers. Please contact our office at (989)275-3668 and we will be happy to verify your coverage before your visit. If you do not have insurance coverage we also offer a variety of services at a discounted rate, ask us what services we offer.
